Pneumothorax during laparoscopic surgery is rare but can cause severe complication such as death. A 62-years-old man undergone laparoscopic totally extraperitoneal inguinal hernia repair under general anesthesia. About 110 minutes after CO2 insufflation , the oxygen saturation was decreased, and hypotension, tachycardia, hypercapnia was developed. Right pneumothorax with subcutaneous emphysema was recognized on the intraoperative chest-radiography. The pneumothorax improved after chest tube insertion. Anesthesiologists should be aware of the possible occurrence of
pneumothorax during laparoscopic surgery, and careful monitoring and immediate treatment is necessary.
